A terminal is available for processing your card when you come for your appointment.
This requires an additional 2.75% transection fee for processing.
Insurance Reimbursement
Patient responsibility to pursue. An itemized medical receipt with diagnostic code and procedure code will be sent to you after your PT session. This superbill can be used for submitting a self-claim to your insurance company.
